Understanding Hyperpigmentation and Common Skin Conditions in Melanin-Rich Skin

Hyperpigmentation is a common concern for those with melanin-rich skin, like individuals from South Asian, African, and Middle Eastern backgrounds. It can often come hand-in-hand with other skin conditions. Here's a breakdown of the most common hyperpigmentation of people with darker skin tones.

Common Hyperpigmentation Disorder on disorders and how to treat them, tailored to the unique needs.

Post-Inflammatory Hyperpigmentation (PIH)

  • Cause: Appears after skin inflammation or injury, such as acne, eczema, or an insect bite.
  • Appearance: Dark patches that stick around even after the initial issue has healed.
  • Treatment: Sunscreen is crucial to prevent the dark spots from getting worse. You can also use topical treatments like niacinamide, azelaic acid, and stable forms of vitamin C to fade the spots. If needed, laser treatments may help, but these should only be done by professionals experienced with treating skin of color to avoid worsening the pigmentation.

Melasma

  • Cause: Often triggered by hormonal changes, especially during pregnancy, and worsened by sun exposure.
  • Appearance: Symmetrical dark patches, usually on the cheeks, forehead, or upper lip.
  • Treatment: Consistent use of UV and blue light protection is key. Topical treatments such as niacinamide, vitamin A derivatives, and combination laser treatments can help, but again, these should be done by a professional experienced with skin of colour.

Acanthosis Nigricans

  • Cause: Commonly linked to insulin resistance, diabetes, and obesity, but it can also appear without any clear cause.
  • Appearance: Dark, velvety patches typically on the neck, armpits, and groin.
  • Treatment: Managing the underlying cause (like improving insulin sensitivity or weight loss) is key. Topical treatments such as retinoids or lactic acid can help smooth and lighten the skin.

Dermatosis Papulosa Nigra

  • Cause: A harmless skin condition caused by sun exposure and genetics.
  • Appearance: Small, dark papules on the face and neck.
  • Treatment: Treatment isn’t usually necessary, but options include removal through methods like scissor excision, cryotherapy, or laser treatment.

Pseudofolliculitis Barbae (Razor Bumps)

  • Cause: Caused by curly hair growing back into the skin after shaving.
  • Appearance: Inflammatory, dark bumps typically on the jawline and neck.
  • Treatment: Shaving less frequently or using a single-blade razor can reduce irritation. Topical corticosteroids, retinoids, or benzoyl peroxide can also help. For a long-term solution, laser hair removal may be the best option.

Effective Treatments for Hyperpigmentation in Skin of Colour

Treating hyperpigmentation in melanin-rich skin needs special care to avoid irritation. Here are some of the most effective treatments:

  • Niacinamide: Reduces melanin transfer and brightens dark spots without irritation.
  • Vitamin C (Stable Form): An antioxidant that fades pigmentation and gives skin a healthy glow.
  • Bakuchiol: A gentler alternative to retinol, it helps improve skin texture and fade pigmentation without harsh side effects.
  • Hyaluronic Acid: Keeps skin hydrated, which can support the effectiveness of other treatments.
  • Retinoids: Boost skin cell turnover and fade dark spots, but they must be used cautiously to avoid irritation in melanin-rich skin.
  • SPF and Blue Light Blockers: Sunscreen is essential to stop existing pigmentation from getting darker and to prevent new spots.
